[REDACTED] and [REDACTED] were re palletizing about 10 pallets of [REDACTED] the [REDACTED]. No issues or complaints during the work period.
What happened
The following day [REDACTED] woke up with a protruding bump on his back. He came to work and wasn 't in pain but we made the decision to send him to Urgent Care for a check.
Injury or illness
[REDACTED] never followed up on the MRI and scans so undetermined. It was discovered that he had a substance abuse problem after this occurrence. We helped him get to rehab saved his job and while at inpatient rehab facility he was thoroughly checked and cleared to return to work. He did return to HCG after a 30 day inpatient program but ultimately relapsed and failed to show up for work.
