Before the incident

Two day work event in [REDACTED] walking from van to restaurant for gathering of caregivers. This was not mandatory rather company paying for meal outside of hospital setting. On phone getting directions to restaurant and walking when I tripped on a rain gutter on road.

What happened

Parked van received phone call from manager wanting to know if I could see her waving at me to see where the restaurant was located. I walked onto road and knew I was put to far and needed to move my van to another location. I turned to go back to van and tripped on a wide cement rain gutter while still on phone I said I just fell and was asked if I was ok. I did go to restaurant and then the pain started to increase. Went back to hotel took [REDACTED] and went to sleep. [REDACTED] called manager that I would not be attending meeting instead going back home and also made a Doctors Appointment with my physician for [REDACTED] to be seen. x-rays showed [REDACTED] Bone Brokien set at time but required surgery [REDACTED].
