What happened
Forklift driver left a skid in a blind spot and didn 't let the material handler know. [REDACTED] came around the corner and tripped over the skid. She fell awkwardly and hit her head back left hand something fell on her right wrist and she has pain in both her legs (suffered contusions). Pain at the time of incident was 8.5. I got her 4 ice packs and 2 over the counter pain pills right away. She took the medicine applied ice packs and I let her rest for 50 minutes. After this first aid her pain was a 4. (Cervical strain bilateral shoulder and upper extremity pain contusions)
