Before the incident
Per EE report I was helping a patient up to the bathroom. After she got off the toilet she walked over to the sink to wash her hands. She said she started to feel dizzy then her legs started to give out on her. I quickly went over to patient to help guider her down to the floor but she was dead weight. The PCA and another RN then came in to help get the patient off the floor and back to bed. We had to do it quickly because patient looked like she was going to pass out again.
