Before the incident
EE was preparing her equipment for the lab draw and sanitized her hands before the lab attempt of the patient.
What happened
The patient was having his blood taken for labs on his hand. EE cannot recall which hand was used for the labs. EE was using a 23 gauge butterfly needle for the lab draw. She had initially used a 20 cc syringe but switched to a 10 cc syringe to obtain more blood for the labs with her left hand. Then she switched the 10cc syringe to a 5 cc syringe to gather the rest of the blood needed for the tests. EE then placed her right middle and index finger above the needle site to place a gauze pad. At this time she pressed the safety on the butterfly needle with her right thumb. The safety did not activate so she pressed it a second time with her right thumb and the safety retracted the needle but she felt a pinch on her right middle finger. EE then removed her glove and noticed there was blood on her right middle finger; EE states the glove had no tears or signs of a puncture. She washed her hands with soap and hot water for 3 minutes and the bleeding on her right middle finger stopped.
