Employee was unloading a trailer with a forklift.
What happened
Trailer did not have parking brakes set and wheel chocks were not in place. Once employee entered trailer and began moving pallets trailer started to move forward. Employee began to back out and forklift fell out of trailer.

Employee complained that he hit his head on the forklift and had whiplash. Update from Injury Activity Report it was noted that the employee had a contusion of C-Spine and Lumbar Spine.
