Before the incident
Worker was performing assembly work on top of lube truck product tanks climbing on structural supports. When morning break buzzer sounded worker removed hard hat and safety glasses and laid them on tank (worker contests he left glasses on) but carried bundle (several dozen pieces) of zip-ties in hand as he climbed down from tank.
What happened
Worker slipped and face came down on up-facing narrow end of zip-tie bundle being held in hand. Zip-ties scratched both (L & R) eyes. No minor first aid attempted or performed on-site. Worker was immediately transported to Urgent Care.

Bilateral Corneal Abrasion and Bilateral Conjunctural Abrasion - both eyes.

Plastic zip-ties.
Summary line
