Before the incident
On [REDACTED] worker had been performing normal welding fabrication duties.
What happened
Worker communicated to supervisor that her left knee popped and now had some discomfort. Worker declined being seen or treated by the company allocated primary care Occ-Med facility at that time. Supervisor and worker entered the occurrence in the company minor injury report log. Worker returned to her normal duties.
Injury or illness
On 11 07 2023 worker communicated to Safety Mgr that she had been seen by an Orthopaedic provider in Spokane WA on 11 2 2023 for evaluation at which time the attending PA-C provided a general work release for 11 03 2023 to rest the effected knee. Worker communicated that the Ortho PA-C told her that he suspected a possible tear and next step would be to order an MRI then most likely surgery to correct with a lengthy recovery. The 11 07 2023 verbal communication from worker to Safety Mgr was the first identifying an actual injury. Seeking medical without notification is outside of company policy and company was not presented with any documentation that she was seen or diagnosed by a suitable provider. Employer scheduled a diagnostic visit to company-partnered urgent care Occ-Med facility (NW Urgent Care) for 11 15 2023 and suggested to worker that she attend this appointment to seek medical attention as per company policy.
Object or substance involved
Unsubstantiated. Update: Provider determined affliction is genetic hereditary - not work-related. Worker would not release supporting medical documents from PCP to employer as-of 12 28 2023 to classify as non-work-related.
