He called S.O.S. from the car and ambulance and tow truck were dispatched to the scene. He was complaining of pain on his right side and wanted medical attention. Ambulance was going to take him to near by ER. This was reported to [REDACTED] and the HR Support team on [REDACTED] at [REDACTED]. [REDACTED] given a DWC-1 and the Well Comp Information. They have returned a partial filled out DWC-1 form. The employee was sent to a hospital for treatment.
