Employee States Employee states that she was in the freezer to get boxes of cookies She was moving boxes out of the way to get the one she needed She placed a box down on the shelf and then her right foot slipped away from her towards the right She was not pivoting just standing directly in front of the shelf at the time of the slip She was able to regain her balance and did not fall to the ground [REDACTED] Manager was present in the freezer at the time of the incident The employee felt immediate pain in her right lower back down to her right hamstring Employee states that she did not observe visible ice or liquid on the floor however the floor was very slippery as though there was ice [REDACTED] loaded her cart for her and the employee returned to her work station [REDACTED] [REDACTED] approached the employee to complete an incident report While completing the incident report the employee noticed that she had also developed pain in her right foot Medcor was called reference 26848430
