[REDACTED] helped stop the bleeding and bandaged her up. [REDACTED] asked if she wanted to see anyone but [REDACTED] said she wanted to go home and use a PTO day. [REDACTED] and [REDACTED] walked her to her car and [REDACTED] confirmed she would be okay to drive home. This was reported to [REDACTED] in HR on [REDACTED] at [REDACTED]. [REDACTED] was given a DWC-1 and the Well Comp Information via email [REDACTED]. They have not returned the signed DWC-1 form. The employee initially did not want to go through workers comp but changed her mind. The employee was seen by her personal doctor and will be going to Concentra on [REDACTED] for treatment.
