Nails in the split cover were not where they were supposed to be.Employee drilled a hole to fit her hooks when she picked it up with hoistthey were not in the typical v-shape. Employee went to set them down ontop of box the right side went in fine but the left side would not. At thatpoint employee tried to push it in with her left hand and her left pinkie got caught under the lip.
