Attaching a 12-ton clamp to material so it could be moved.

When the clamp was placed on the material the clevis that was attached stayed in an upright position. When the worker went to close the clamp the movement caused the clevis to fall pinching his left index finger between the clevis and the clamp. Worker removed his glove and noticed blood and swelling.
