What happened
The employee was shearing golf molds while leaning on the distance table instead of using it for distance as required he used his left hand to push the molds into the shear to cut them in half. The employee did not use the provided channel locks to hold the mold as directed. When pressing the foot peddle to operate the shear his finger was still in the cutting area and the shear came down on his left index finger tip.
