What happened
Improper build - not following build instructions - spun wall bay and brought it off balance on table. Other employee was squatting by it using mallet and she did not have control of fixture. [REDACTED] knew table was unstable and took her hands off the fixture and it fell back off the table it was being built on. There is another work table right behind that so the fixture didn 't fall entirely on ground.

Lynne made no claim of Injury until 9 27 23 then went to the doctor for shoulder pain.
