What happened

On [REDACTED] after lunch break @[REDACTED] [REDACTED] was instructed to clean and organize the hangar facility with another employee. [REDACTED] began with sweeping hangar floor [REDACTED] then asked [REDACTED] Ops MGR if the spare file cabinets on the hangar floor needed to be removed and discarded. [REDACTED] pointed out the damaged cabinets that could be discarded in the dumpster located on the employee parking area. [REDACTED] proceeded to retrieve the forklift to remove the cabinets (7-8 cabinets) AMT placed four to five cabinets on a wooden pallet. [REDACTED] then proceeded to use the forklift to transfer the cabinets to the dumpster via a locked SIDA gate. Once at the dumpster [REDACTED] lifted the pallet with the forklift with approx 4-5 cabinets up high to the lip of the dumpster. While holding dumpster lid open he used his other arm to push the cabinets off the pallet stretching both arms out in an awkward position. The weather was windy that day. [REDACTED] repeated this on
