Before the incident
Employee was applying mastic at roof leading edge to install starter course of shingles. Had a bucket of mastic a trowel and starter shingles. Saftey harness was on attached by lanyard to fall rope.
What happened
Employee loosened his lanyard slack to retrieve materials from another part of the roof. Upon return to the work location the employee failed to readjust his lanyard length to account for the new fall risk height.

Employee broke both arms and received a facial injury. One of the elbows required surgery.
