Before the incident
Operator in training turned around towards the weld table in the [REDACTED] weld booth after offloading an axle that was done being pre-welded onto the conveyor roller table next to [REDACTED] weld booth.
What happened
[REDACTED] was loading an axle from the rollers with the yellow hoist. Trainer did not see [REDACTED] standing there and started grinding. Sparks from the grinder bounced off the surface of the inside of trainee 's safety glasses and into his eye resulting in a foregin body to his eye.
