Before the incident
Employee was training on new equipment.
What happened
Trainer was holding transfer arms (with large adjustable wrench) open to install shim that would allow the opening closing lever to be adjusted. The installed shim fell out and trainer decided to go ahead and loosen the bolt (left-hand thread) holding the transfer arm to the lever shaft. At the same time the trainee attempted to remove the line-up plug from the fixture. When the tension on the bolt released the transfer arms slammed closed (spring return) and the tip of trainee’s thumb was pinched in between the transfer fingers and line-up plug.
