Loading trailer
What happened
Employee was unloading and loading trailers in the shipping Cooler. During this time the employee opened the bay door #2 for the Cooler and started to lower the lift gate attached to the trailer that was parked on the door. (The gate must be lowered before loading or un-loading trailers because it blocks the door of the trailer). While lowering the lift gate the gate had a hydraulic line with a catastrophic failure and it burst. This caused the release of pressure and the heavy gate fell and struck the employee in the hip and side. The employee then proceeded to the Shipping Supervisor office to report the injury. The employee was transported to the ER of his choosing to be treated due to pain from injury. Update - [REDACTED] Employee sustained injury to lower back as result of incident. Update - Employee was removed from Restricted duty as of [REDACTED]. Employee returned to full duty.
