Before the incident
The driver was dropping a trailer at a customer. She had unhooked from the trailer and began to pull forward when she heard a noise and looked back toward the trailer.

She had forgotten to un-hook the airlines to the trailer and as she pulled forward the airlines broke and came back and hit the rear window of the truck breaking the window. She was still looking back when the glass broke causing a piece of glass to get into her R eye.
