Before the incident
A warehouse operator was dropping a slurry tote at [REDACTED] solution drop off area. The operator 's foot was slightly sticking outside of the running lines of the PIV. When the operator went to set the tote into position their foot drug across the side of a tote that was located to the operator 's left. Their foot became caught between the PIV and the tote resulting in the operator 's foot bending back and causing a fracture.
