Before the incident
Employee was working on a receiving task. Received material in placed it on the conveyor belt and then the material got stuck towards the top end of the conveyor belt. Employee walked over to get a better look at the stuck material.
What happened
Material was caught on conveyor belt near the top end employee went to go check and look and her eyes wandered away from the floor and tripped on a pallet. Her left foot tripped on the corner of the pallet and misstepped on the right leg causing a sprain in the right ankle.
