Before the incident
TM climbed fixed ladder to deenergize process equipment
What happened
TM slipped and fell approximately 4 feet while climbing down fixed ladder. Landed hard on both feet then fell to the floor. Did not hit head on anything but TM was disoriented and reported back pain after the fall. [REDACTED] contacted EMS. EMTs arrived and assessed TM. Ruled out major medical issues and recommended TM go to ER for further evaluation. TM declined and eventually left work early due to back pain.
