Employee walked from the Loading Dock room through the garage door and into an area that pallets are typically placed located off the end of production. Employee was not paying attention to forklift traffic and was looking away from traffic trying to get the attention of another employee that was up on a higher level.

Employee walked out of the loading dock room not observing the forklift traffic and stood in a spot drivers typically sit pallets. The path and timing of employees movement and subsequent standing in an area reserved for pallets coupled with the other forklift operators movement placed the employee in a blind spot to the forklift operator during the entierty of the time Employee walked through the garage door to the spot employee stopped at. The other forklift operator placed the pallet in the designated spot while doing so pushed employee forward with the pallet.
