Before the incident
Prior to the injury happening the employee was operating the punch press.
What happened
While running the punch press the machine failed to cut through a wire completely. The wire hit the backstop that assists the wires to drop into the wire cart. Due to the wire not fully cutting the machine kept running building up pressure causing the wire to bow. When the operator saw the issue at hand he grabbed the wire to try and pull it away from the backstop to relieve the tension. While doing this the wire broke off at the failed cut point releasing the wire tension and bouncing off the backstop and hitting the employee in the arm.
