What happened
Employee was working from the third step from the top of a step ladder mounting a hose reel on a overhead platform. As he reached forward to tighten a bolt his feet slipped off the step causing him to fall about three feet down the ladder toward the floor. He landed upright with his left foot on the second from the bottom rung of the ladder. This caused a lumbar strain on his left side.
