Monitoring a Transfer on Bariatric Patient

Mrs. [REDACTED] room assignment 723W(window). I was putting a bariatric patient back to bed and she transfers herself to the bed with little or no assistance at all. While she was transfering to the bed she fell weak. She got up from the wheel chair and was about to sit down. Half of her body( the right side) was sitting on the bed and the left side was hanging off the bed. I assisted the patient and lifted her (left side) in the bed to keep her from falling off the bed. Patient was safely placed in bed no injuries to patient. I felt the pain but thought it was minor. I went home applied lidocaine to back and woke up in major pain in back and right wrist.
