Unhooking lifting hook from container.

Employee lifted the plug bin into the scrap hopper and set it down inside the hopper. Once down the chains attached to the side lifting lugs (see pictures on following page) were removed and the chains were attached to the bottom lug to lift the plug bin from the bottom and tip the contents into the hopper. When the employee went to remove the chain from the bottom lug and reattach the chains to the side lugs he noticed one of the chain 's hooks was twisted. He reached in to grab the chain and adjust it but in doing so the scrap hopper tilted slightly which caused the plug bin to shift causing the chain to go taught. When the chain went taught the employee 's finger was pinched between the steel lifting lug and the hook cutting off the tip of his finger. Employee was wearing gloves at time of incident. The locking mechanism for the scrap hopper was supposedly damaged the day before but had not yet been reported
