[REDACTED] was at customer location. He went to pull up the dock plate at location. When he grabbed the bar to pull the plate his feet were wet. He slipped and fell forward. He ended up falling off to the side and off the dock. He fell from dock height and landed on the ground outside. When he hit he slammed his head on the ground causing a cut that started bleeding he also landed on his hands to catch his fall causing deep abrasions and cuts.
