What happened
Employee went to use his forklift to open [REDACTED]. Employee failed to unlatch the door before trying to lift it with his forks. When the employee lifted the door with the forklift it caused the bottom panel to stay latched and the rest of the the panels broke off upward. Without consulting anyone the employee then went to unlatch the door. The bottom panel then snapped upward hitting the employee in the forehead resulting in a deep laceration that required stitches.
