What happened
MP [REDACTED] forklift operator was traversing the dock plate to access the railcar to unload product. The mast of the PIT was caught on the dock plate due to being too low causing them to impact the dock plate upon entry. The impact caused the fork truck to stop abruptly. The operator fell forward striking his head on the interior of the roll cage of the fork lift. PIT was found with wear on the tires
