Worker was a maintenance associate fixing the door of a melting furnace that wouldn 't open close properly. The door was malfunctioning due to a riser that got stuck under the door cylinder

The worker did not have the equipment properly locked out. So when the riser was removed the residual pressure in the cylinder allowed the door to open causing the counterbalance to come down and impact the worker 's head.
