Patient was placed on 1:1 for being a danger to self. per. per hospital policy the patient door should be open and lights Turned-on whenever the patient is in the room. The patient started yelling at monitoring staff and refusing lights to be turned on or door being open. monitor staff called this RN to intervene. RN tried to explain to the patient that the door should be open and lights turned-on to ensure patient was safe. This RN turned on the lights and opened the door. patient started slamming the door multiple times and making verbal threats to assault staff. PERT and security paged and tried to assist with no success and left the ward. The patient continued to slam the door and was irritating other patients. This RN tried to deescalate and redirect patient to stop slamming the door and allow staff to open the door. that 's when the patient rushed and attached this RN. patient grabbed me dropped me on the floor and continued to kick scratch and punch.
