[REDACTED] was in Medline ready to take her medication pt then left the Medline and refused her AM meds during med line. Reapproached pt later in the hall with another staff member MHT to assist me. [REDACTED] grabbed the med cup and glass if juice threw it at me then proceeded to hit my head and left side of face multiple time with her hands ripping off my glasses. Other staff came to help me.
