What happened
A team member was working at the exit table of a tube mill when a pipe became jammed. Another tube was being processed through the mill simultaneously which caused both pipes to get stuck on the rollers. The tension generated by this situation caused the second tube to kick off the rollers and hit the team member who was partially struck by the tube. As he tried to move away he lost his balance and fell on his rib inside the containment area.
