Before the incident
Employee was performing the task of washing down the Cogen building He completed the 5th floor and made his way to the 4th floor to tie in his washdown hose There wasnt an adapter on the connection point therefore the employee returned to the 5th floor with a spanner wrench and stated the adapter was tight on the connection As he was applying force to the spanner wrench the wrench slipped and he lost balance He caught himself on the grate and at that moment felt tension to his lower back He immediately reported the incident to his supervisor was evaluated by onsite EMT and left the site for evaluation by outside medical
