[REDACTED] [REDACTED] about [REDACTED] Employee went to valve pit to look inside to see if they could find any leaks after RU pumped it out Employee opened the large double doors but not the inner grated safety gate They took hard hat off so it wouldnt fall off and into pit Employee laid on belly on the concrete but didnt see any evidence of leaking Employee went to one of the smaller doors and opened it while standing on ground outside of concrete They then opened the inner grate door and pushed it back to where they thought it locked in the upright position Employee then looked into the opening and immediately was struck by the falling inner grate door It was a glancing blow Employee knew they were cut and bleeding Employee did not lose consciousness or had any dizziness Employee walked to gate 2 and called [REDACTED] while walking
