Before the incident
There was a problem with the string turn up system where they were not able to turn up From there they put the sheet into the dry end pulper After the turn up system was fixed they tried to rethread the reel They had the tail going to the reel and had the sheet halfway widened out when it broke between the calender and the reel At that point the sheet blew up in the air so the employee opened the calenders in order to get it to drop back into the dry end pulper The employee also moved the cutter to the front but it stalled about four feet from the home position The cut portion of the sheet piled up on the threading conveyor and railing of the dry end pulper which eventually led to the whole sheet piling up The chute was plugged causing that From there the calenders were stopped and efforts to unplug the dry end pulper started They opened the calender stack and fed the sheet into the opening of the stack and then down into the pulper as you cannot push the sheet down at that point This
