Before the incident
The mill was down on a scheduled outage and the Top Making Felt was being installed The felt is pulled into position by attaching a rope to felt leader and then using an overhead crane to pull it through the felt run Early in the process the rope appeared to have more tension than expected so the crew stopped and took corrective actions to adjust press roll positions to remove restrictions This allowed the felt to pull easily through the path with no appearance of extra tension on the rope However the rope broke and tension acted as stored energy causing it to whip back and strike the Team Member
