Before the incident
The employee was attempting to remove nails from the bolt holes as the skid was being lowered.
What happened
Employees were setting the main fuel gas skid into position on the southwest side of the boiler. As the equipment was being lowered down onto the bolts on the piers the injured employee stated that he noticed nails in the bolt holes on the north side of the skid base plate and tried to remove the nails. In the process of trying to remove the nails the employees index finger was pinched inbetween the equipment base plate and the anchor bolt.
