What happened

Employees were tasked with dismantling modular scaffold on the North side of the Utilities unit located on [REDACTED]. Two employees were on top dismantling the modular scaffold while four employees were in the daisy chain to pass down dismantled material to employees that were stationed on ground level to stack the material into designated scaffold racks. One employee at the top level removed what they thought was a tenfoot modular scaffold leg attached to a saddle clamp and began to pass the material down to the employee below in the daisy chain. Unknown to the employee the tenfoot leg was in fact a sevenfoot and threefoot leg stacked on top of one another. When the employee passed down the assumed tenfoot leg and saddle clamp the bottom sevenfoot section came apart from the top threefoot section causing it to fall to grade level. When the sevenfoot leg and saddle clamp fell to grade level the material came into contact with an employee who was positioned at grade level. The material
