Driver was driving back to [REDACTED] from [REDACTED] with a Tool (approx 2000lbs) in the back of the Heavy duty pick up truck On [REDACTED]). He did not have the load secured when he left the [REDACTED] customer.

Driver stated he slammed on the brakes the tool slid forward in the bed of the truck striking the back of the bed bending it into the cab of the truck. The cab then bent the back window shaddered and the back of the cab pushed forward into the right side of the drivers seat. At the time he called and stated he was 'fine ' the and drove back to [REDACTED]. He complained of shoulder pain when he returned to the shop (approx. 5 minutes post arrival) He left and drove himself to the emergency room near his home.
