na attempted to get patient off of toilet by herself was unsuccessful na asked writer to assist and a [REDACTED] was taken into room writer was to the left side of patient aide in front of [REDACTED] we advised the patient on the count of three to stand patient did not seen to help we attempted to stand patient again into steady and patient was like a box of rocks and from the movement of trying to lift her and her not help almost as if she held herself down writer immediately noted I had hurt my back switched sides with [REDACTED] this was not my patient and I was unaware that the patient was doing this as a behavior per her nurse when I reported to her writer reported it immediately to [REDACTED] Supervisor she printed sheet out for me and advised I call the ouch line ouch line sent me down stairs to ER
