pt was being transferred from wheel chair to reclining chair. pt had on grippy socks gait belt and walker in front of her. As we were transferring to recliner she had pivoted her back side to chair. Recliner chair was not locked but had been setup by PT. As she pivoted she said she was getting dizzy and lightheaded. I called for help because she had passed out and I was left trying to get the rest of her body in the recliner. I was assisted by RN and an occupational therapist to help me with getting her in recliner chair. She had previously worked with PT with no issues.
