What happened
[REDACTED] got pt up to chair. [REDACTED] did not tell me how. I asked the pt who stated he used a walker and turned pivoted to bed. Gait belt in place socks on pt walker in use. When pt stood up and started to pivot he was too weak to stay standing. I had to grab him and hold him by the gait belt his full weight and pivot him to bed. [REDACTED] called afterward to tell me he was minimal assist after I had gotten pt to bed.
