[REDACTED] is not sure where the injury occurred it could have happened in at least two operations on the Sweeper assembly line based on information from [REDACTED] and the Supervisor ([REDACTED]). 1. Engine Compartment Tank Marrying operation: [REDACTED] grinds the tube ends places the short and long tubes and other components on the fixture welds the tube weldment checks for leaks uses a hoist to remove the tube weldment from the fixture and places it on a rack. 2. Engine Compartment Step 5: [REDACTED] uses a hoist to lift and place the tube weldment on the fixture. This fixture placement is above [REDACTED] 's head when standing on the floor a step platform is required and available.

What happened

. Engine Compartment Tank Marrying operation: Most if not all welders have been lifting the shorter tube manually and placing it on the fixture. In [REDACTED] 's case due to his height he has to use his shoulders to raise the tube to place it on the fixture. This is poor lifting technique and the weight lifted (short tube = 63 lbs.) exceeds the physical demand requirement of lifting carrying up to 50 lbs. repeatedly per the Welder Level 1 - Production Welder job description. 2. Engine Compartment Step 5: Per [REDACTED] 's was trained incorrectly on how to place the tube weldment on the fixture. This technique is poor per [REDACTED].
