Before the incident
The accident happened at [REDACTED]. I [REDACTED] came out of [REDACTED] office to grab the headlamp. [REDACTED] called me and told me that a belt needed to be changed from the back of the oven. [REDACTED] drove the cart to the back of the Paint area behind the oven. area. [REDACTED] shut the oven off. [REDACTED] opens the front door and then closes it (Oven). [REDACTED] went to the right side where we walked to the back side of the oven. As we walked to the back [REDACTED] was shutting off the switches. We both waited for the motor to shut off completely before we started to work on it and remove the cover. I grabbed flat head screwdrivers and went to remove the two screws that were held on the guard for the belts. We looked at it with a flash drive and seen all the 3 belts were damaged and old. I removed all 3 of the belts. [REDACTED] had new belts hanging and we both started putting them on. After we put them on we tried and tried to adjust them.
What happened
After [REDACTED] was not able to adjust them I [REDACTED] was trying to adjust the belt and the machine turned on and my hand went through the belts. My left hand went up and over to the right clockwise. When it turned on I saw something fly and I did not know it was my finger. I did not know how to turn the machine on or off. No one knows who turned it on or how it turned on. After that happened [REDACTED] went into shock and he was just standing there and yelling. [REDACTED] appeared and he got a rag and told [REDACTED] to call the ambulance. He wrapped a rag around my wrist. [REDACTED] took me outside. Do you know how to Lock out Tag Out? [REDACTED] said “It was a quick thing. I did not think anything of it. I know how to do it but I did not turn it off.” What fingers did you injure? Left Hand Index Finger - Chopped off in 45 degree Middle Finger - Cut off 90 % off the tip was cut off and 10 % was attached ring finger - a slice of it and the nail bed shifted.
Injury or illness
it is a partial amputation to the distal phalanx of the right index finger. Ring finger just laceration. Left long finger laceration Dr. Clark mention before the surgery that the distal interphalangeal joint is intact.
