What happened

[REDACTED] was fabricating [REDACTED] on the bender machine 2. The machine once the cycle has started will center the material with the clamp guide that will raise and adjust to the proper centering before bending into shape. The material is bent into shape by way of the motorized swivel arm that adjusts to the length of the material being bent. The machine is cycled by a foot pedal and the material must be held steady in the material shaft by an operator prior to starting the cycle. [REDACTED] placed the material in the machine on one end while holding it steady for the centering clamp before starting the cycle on the other end. While doing so he stepped on the pedal which initiated the cycle. His right fingers became caught in the centering clamp guide and his right arm became compromised by the motorized swivel arm that bends the material. The machine is guarded with a disengaging safety matt which stopped the machine as soon as [REDACTED] stepped on it. The cycle did not complete b
