EMPLOYEE USED A HOOK EXTENSION TO BRING ROTOR CLOSER AS SHE BENT DOWN AND REACHED IN TO PICK UP THE ROTOR SHE STOOD UP WITH ITEM THINKING SHE CLEARED THE SUPPORT BEAM TO THE SHELF ABOVE AND SMACKED THE BACK OF HER HEAD.
